Human Reliability Analysis using a Human Factors Hazard Model

Human Reliability Analysis (HRA) has found application within a diverse set of engineering domains, but the methods used to apply HRA are often complicated, time-consuming, costly apply, specific particular (i.e., nuclear) applications, and not suitable for direct comparison amongst themselves.
 This paper proposes Factors Hazard Model (HFHM), which builds an method from tools Fault Tree (FTA), Event (ETA), novel model considering serial Error Probability (HEP) more relevant psychomotor-intensive industrial commercial applications such as manufacturing, teleoperation, vehicle operation. The HEP approach uses Performance Shaping (PSFs) human behavior, well characteristics unique system architecture its corresponding operational behavior. HFHM tool is intended establish common analysis approach, simplify automate modeling likelihood mishap due human-system interaction during hazard event.
 executed software (MS Excel SysML) that trade sensitivity studies can be conducted iterated automatically. results generated by guide risk assessment, safety requirements generation management, design options, controls architecting process. Verification evaluation through simulation subject matter expert illustrate value in key applications.
